4C Food Program (An equal opportunity provider)

Child and Adult Care Food Program (CACFP) through United State Department of Agriculture (USDA).

The 4C Food Program services six (6) counties: Milwaukee , Waukesha, Washington , Ozaukee, Racine , and Kenosha . The program provides nutrition education and reimbursement for Family Child Care providers who serve meals which meet the USDA Child and Adult Food Program guidelines.

Providers are visited three (3) times per year by a 4C Food Program Field Representative. The Field Representatives check food served, sanitary, and basic health and safety conditions of the programs that they monitor.

In 2006, we serviced 7,000 customers. This means that approximately 50,000 children were served nutritious meals that met the guidelines as set forth by the USDA.

 The 4C Food Program offers…

  • Reimbursement for meals served that meet USDA guidelines
  • Free nutritional training
  • Free incentives given during home visits
  • Free envelopes to mail Food Program forms to 4C
  • Free Record Keeping calendar
  • Quarterly Newsletter
  • Food Program claims processed locally.
  • Voice-mail system in place to contact your Food Program Field Representative during off-business hours. Convenient way to leave requests for forms, etc.

Family Child Care programs are eligible to be part of the Food Program when they are regulated as a Certified or Licensed Provider, and have at least one enrolled full- or part-time child
